This form is used by existing EMDR Europe Accredited Practitioners to apply for re-accreditation.
Re-Accreditation Application
Each declaration has a Yes / No toggle. If any answer is No, an explanation field is required.
I maintain my professional registration and am in good standing with my accrediting body.
I have no criminal record that would prejudice clients or bring EMDR Ireland into disrepute.
I have not been dismissed or refused membership of a professional organisation for misconduct.
I will notify EMDR Ireland of any complaints, disciplinary proceedings, or changes to my professional standing.
I hold adequate professional indemnity insurance for my clinical practice.
I agree to abide by the EMDR Europe Code of Ethics and the rules of EMDR Ireland.
I agree to engage in regular EMDR consultation and continuing professional development (CPD).
